The DIAMOND program uses a collaborative team comprised of the primary care physician, a care manager and a consulting psychiatrist to more frequently stay in contact with, and better coordinate and manage the care of, a patient with depression. Health plans are paying clinics to provide these additional services. DIAMOND is currently getting 5-10 times as many patients into remission by six months compared to patients receiving typical primary care for depression.
