Dear Sir,

I am composing this letter to express my deepest gratitude to the innovative program called DIAMOND. The program is set up with a care manager who liaisons between the patient’s medical doctor and a consulting psychiatrist. The patient is in regular contact with the care manager, and is free to express any feelings or situations that are causing problems in their life. This liaison serves as a “sounding board” to get your problems addressed by other health care professionals if necessary. The DIAMOND care team discusses your case together in a timely manner to arrive at an appropriate solution or plan of action.

My DIAMOND care manager gained my trust and made me feel like someone was there for any reason needed. She convinced me there is someone in the health care profession that not only treats me in earnest and really cares, but who gets results very fast. She has been there for me through my mother’s death, and the euthanizing of my pet. And, through the care team consult, secured a prescription to ease my anxiety and emotional distress. She has been a big asset in helping me cope with my stressful situations. Behind the scenes I knew both my doctor and the psychiatrist were there and informed of my situation as well.

My care manager may have saved my life from suicide. Thank God I had her to call when this unbelievable experience happened to me. I was on the edge; I thought that my only hope was calling her. She talked me through the situation, convinced me into going to the hospital, and while doing so had an assistant call my daughter to drive me and notified Regions Hospital that I was coming in as a possible suicidal patient. I am now out of the hospital and at home happier than I have been in years. I have since seen both my physician and care manager and have resumed my care under DIAMOND.

My daughter said my care manager and the rest of DIAMOND team make up one of the best health care programs she has ever seen or heard of. I agree whole-heartedly and am confident that Park Nicollet will keep and expand DIAMOND. I can foresee great acclaim for this program and expanding it to other health care organizations in the future.
