This work flow guide is to provide support to ambulatory care clinics implementing MDH's health care home standards and criteria. The document demonstrates and discusses the clinic flow for care coordination, billing and denial process, and parallel processes that occur in a certified health care home. The key components of the State's health care home standards and criteria are addressed throughout the document.
