Care Coordination Referrals “Exploding"
at South Lake Pediatrics
Care coordination has made a marked difference for patients at South Lake Pediatrics. Providers have seen approximately a 23 percent reduction in the number of behavioral health patients requiring emergency department or hospitalized care in 2013 and 2014. “Care coordination has helped us track and break down barriers to success of important referrals,” says Dale Dobrin, MD, Senior Medical Director of the 40-year-old practice. Discussions Continue on Population Health Measurement
As follow-up to previously hosted collaborative conversations around global health measures, ICSI recently convened a discussion with member organizations, MN Community Measurement and other stakeholders to further consider frameworks for population health measurement. Organizations are looking for ways to routinely collect data and identify measures that will help them better understand the health of the communities they serve; challenges continue on how to use that information in clinical encounters, if at all. Colorectal Cancer Screening: Reaching the Never Screened with FIT Tests

On this American Cancer Society webinar, Allina Health System will share its targeted intervention to reach the never-screened by mailing FIT stool tests. The impressive response rates were tracked in the Caucasian and non-Caucasian populations, and expanded from an initial pilot of six clinics to the entire system.
