New Health Care Disparities Report Shows Improvements in Diabetes, Vascular Care

Low-income patients in Minnesota with diabetes and vascular disease had better control over their conditions in 2014 than the year before, according to the eighth annual Health Care Disparities Report conducted by MN Community Measurement and sponsored by the Minnesota Department of Human Services. The report evaluates care outcomes for patients enrolled in Minnesota Health Care Programs (MHCP) on 12 quality performance measures. ICSI member Entira Family Clinics increased the percentage of its MHCP-covered patients reaching optimal diabetes management by 21 percentage points over the past year. Learn more.

RARE Mental Health Collaborative Wraps Up

Team members from the 11 participating hospitals part of the RARE Mental Health Collaborative came together on February 24, 2015 to share their results and lessons learned. As they delivered their final reports, participants talked about signs of progress, data that is just beginning to move in the right direction, successful efforts that will be expanded and others that didn't work quite as well.
