Now Available: TCOC and Affordability Improvement Within the Triple Aim Webinar
This recorded webinar presented by Howard Epstein, MD, chief medical officer, Preferred One, followed by a deeper discussion with Dr. Epstein and Gary Oftedahl, MD, ICSI’s chief knowledge officer, will help you understand imperatives for addressing health care affordability, share examples of successful Triple Aim collaboratives, and explore Total Cost of Care (TCOC) models for our community. Preliminary Results Shows COMPASS
Improves Patient Outcomes
Results through Dec. 31, 2014 show that the team-based COMPASS model improves the outcomes of patients with uncontrolled depression and uncontrolled diabetes and/or heart disease. The aggregated results from the 18 regional medical groups participating in this ICSI-led initiative show the model is exceeding goals set for depression and heart disease improvement, and close to the goal set for diabetes improvement. ICSI Welcomes Two New Members
The membership committee of ICSI's Board of Directors has approved Axis Medical Center and Neighborhood HealthSource as provisional members. Both are located in Minneapolis and are part of the Minnesota Association of Community Health Centers. They are currently implementing quality improvement activities to qualify for full membership.
