Following passage of the 2008 Health Care Home legislation, the Minnesota Department of Health (MDH) and Minnesota Department of Human Services engaged ICSI to help develop the state-mandated health care home model. ICSI formed a steering committee and held a forum to engage key stakeholders as they worked together to create a model that would be widely adopted by providers, supported by health plans and purchasers, and used by patients.
$1 Billion in Savings
In February 2016, results of a University of Minnesota evaluation of health care homes were announced. Researchers found that over a five year evaluation period, the model – emphasizing collaboration among care providers, families and patients – saved Medicaid and Medicare approximately $1 billion.
