ICSI works with Minnesota health care practices to build their capacity for continuous quality improvement and their ability to implement new evidence-based treatments and bring health service models into practice. SIM Practice Facilitation
ICSI and Stratis Health have combined practice facilitation resources to bring together a mix of primary and specialty care provider organizations for quality improvement activities as part of a $45 million State Innovation Model (SIM) cooperative agreement. The Minnesota Department of Health and the Minnesota Department of Human Services received this award in 2013 from the Center for Medicare and Medicaid Innovation (CMMI) to help implement the Minnesota Accountable Health Model. Chronic Condition Management
ICSI, in partnership with the Minnesota Department of Health (MDH), HealthPartners Institute and Stratis Health, is providing program management and practice facilitation for five Minnesota health systems in an effort to improve their care of patients with chronic health conditions.
