Join us for another Mental Health Community Partners Networking event, and continue to learn about bright spots and connect with colleagues toward improving care transitions for people with mental illness.
Our agenda includes information about: 1) CentraCare's Community Paramedics program, 2) A collaborative project between Central Minnesota Mental Health Clinic and CentraCare Health supporting integration of primary care providers in mental health centers, 3) A community effort to address Mental Health Crisis, and 4) Time for networking and small group interaction.
Please note: Funding for this initiative prohibits coverage of food at meetings. Therefore, LUNCH WILL BE ON YOUR OWN. We'll direct you to several great options to grab lunch with a colleague.
Don't forget to join the Mental Health Community Partners Network group on LinkedIn for even more information, tips, and resources.

*From January through August, 2016, we will be hosting a variety of in-person events and subject-specific webinars focused on care transitions for four patient population groups: 1) patients admitted to an inpatient psychiatric hospital unit; 2) patients with acute conditions and known mental health diagnoses; 3) patients with chronic disease who develop mental health conditions; and 4) patients newly diagnosed with serious mental illness.
This project is made possible through the Minnesota Hospital Association’s Hospital Engagement Network, funded by the Partnership for Patients program of the Centers for Medicare and Medicaid Services.
