This seal signifies that the bearer has been reviewed by and meets the criteria of the ICSI Patient Advisory Council (PAC). ICSI's PAC champions patient and family-centered care, increased awareness of the patient/family perspective and utilization of shared decision-making between clinicians and patients, families and/or caregivers.
Criteria
1. Demonstrates relevance to and respect for the patient, family and/or caregiver with appropriate and respectful language.
2. Recommends that early in their care, the patient identify their family and/or caregivers as part of the care team. They are to be informed, included in health care discussions, encouraged to ask questions, involved in decision-making and invited to participate with care, as determined by the patient.
3. Advocates for a coordinated care team model responsible for effective communication with the patient/family/caregiver, thus preventing care fragmentation and medical error.
4. Employs literature searches that includes one or more of the following:
- Patient experiences of the condition, including diagnosis, medication and other treatments, follow-up care and quality of life
- Information needs and preferences
- Participation in decision-making about treatment
- Overall satisfaction with care received
- Patient safety concerns
- Care team models
- Ethnic and diverse population considerations
5. Identifies shared decision-making opportunities throughout guidelines and materials, where appropriate, and decision aids are included or referenced when available.
6. Encourages use of and/or provides examples or resources for patient and family-centered education materials.
7. Includes one or more patients or community members on work groups or committees, or patient/community member(s) input is sought to review guideline or other materials and return feedback.
