A health care home statewide learning collaborative is required by the 2008 Health Reform legislation, Minnesota Statute §256B.0751. This learning collaborative will provide an opportunity for health care homes and state agencies to exchange information and enhance understanding related to quality improvement and best practices. ICSI was awarded the contract to design and initiate a learning collaborative model that is able to accommodate a large number of clinics and clinicians becoming health care homes.
