Following passage of the 2008 Health Care Home legislation, ICSI adopted health care home as a strategic initiative. The Health Care Home (HCH) Steering Committee included representatives from primary and specialty provider groups, health plans, purchasers, patients, academia, and government agencies. A key goal was to create a model that would be widely adopted by providers, supported by health plans and purchasers, and used by patients who stand to benefit from its availability.
ICSI facilitated a forum to explore the status of Minnesota care delivery organizations’ efforts around the HCH concept, which helped engage key stakeholders as they worked together to define and advance this approach. In addition, the Minnesota Department of Health (MDH) and Minnesota Department of Human Services (DHS) engaged ICSI to help develop the state’s mandated health care home model.
ICSI was also awarded another MDH contract to lead a statewide learning collaborative to build the capacity of primary care providers to meet the standards, criteria and expected outcomes of health care home certification and to meet state mandates for health care homes. This contract ended in 2012.
ICSI staff members also served on several MDH/DHS committees and groups working on payment methodology strategies, health care home standards and clinic certification criteria.
