Objectives
At the conclusion of the Colloquium, participants should be able to:
- Identify innovative and successful ways to take a more outside-in approach to connecting and collaborating with local communities in pursuit of Triple Aim goals and fostering healthier communities.
- Describe the evolving value-based care and payment models, on national and local levels and from both the provider and patient perspective, to be better prepared for upcoming changes.
- Recognize the changing roles and essential new skills needed to prevent burnout and thrive as leaders in today’s challenging health care environment.
- Discuss innovative approaches and designs for care delivery that could be implemented in an attendee’s health care organization, especially primary care, including the integration of behavioral health and primary care.
