ICSI Website Gets a Refresh: Share Your Feedback
We've given our website an early "spring cleaning!" The most noteworthy change was to our main site navigation (or content areas), which were renamed and reorganized. The old Health Initiatives section has been divided into two sections that better describe ICSI's current focus areas:
- Health Care Transformation, which includes our work on patient engagement, health care value and health equity.
- Dissemination & Implementation, which encompasses our behavioral health integration, COMPASS and practice facilitation efforts.
- Information about most of our older initiatives was pared down and moved to a new Legacy Work section under About ICSI.
- Learning & Events is the new home for our Learning Center, plus information about the Colloquium and Reinertsen Lecture.
- Our Patient Advisory Council and other resources related to patient engagement and shared decision-making can be found in the Patient Engagement section under Health Care Transformation.

2015 Health Equity of Care Report
Minnesota Community Measurement (MNCM) recently released its 2015 Health Equity of Care Report. In conjunction with MNCM, ICSI was pleased to host a two-part webinar series this past spring following the release of the inaugural 2014 report. The slides and webinar recordings are available here. In the second webinar, ICSI members shared their improvement activities using the race, ethnicity and language (REL) data. Health Care Homes Show $1 Billion in Savings
In February 2016, results of a University of Minnesota evaluation of Health Care Homes (HCH) were announced. Researchers found that over a five-year evaluation period, the model – emphasizing collaboration among care providers, families and patients – saved Medicaid and Medicare approximately $1 billion. At the same time, clinics participating in a health care home model of care outperformed other clinics on quality measures. Learn more about the evaluation results.
Since the passage of the HCH legislation in 2008, ICSI has supported its members and the state of Minnesota in the implementation of this model. ICSI led a statewide learning collaborative to help primary care providers develop the organizational capacity to meet HCH certification standards. ICSI staff members continue to serve on many HCH committees and work groups, currently including the Advisory, Certification and Behavioral Health Home Committees. Learn more about our work.

Welcome New PAC Member
Gladys Chuy is the fifth new member of ICSI's Patient Advisory Council (PAC). Gladys is a quality improvement specialist for Medicine Specialties at Park Nicollet Health Services. She takes pride in working in quality and process improvement. Some of her passions are improving community health through cultural competence and working to decrease the health care disparity gap in Minnesota. Gladys joined the PAC to be a voice for the voiceless. It is important to her that all patients have a voice in the care they receive and are provided quality care.

The membership committee of ICSI's Board of Directors has approved Ortonville Area Health Services as a new provisional member. They serve their community through Ortonville Hospital, Northridge Residence (nursing home), Northside Medical and Clinton Clinics, and OAHS Home Health Agency. They are currently implementing quality improvement activities to qualify for full membership.

Hutchinson Health Physicians Promote Safer Use of Antibiotics
In the February 6, 2016 edition of the Hutchinson Leader, an article entitled Doctors' Advice: Get Smart About Antibiotics featured the efforts of two Hutchinson Health physicians to promote the message that overuse of antibiotics might not only be ineffective, but could be harmful.

New CMS Guide to Preventing Readmissions Among Racially and Ethnically Diverse Medicare Beneficiaries
The Centers for Medicare & Medicaid Services Office of Minority Health (CMS OMH), recently released a new Guide to Preventing Readmissions among Racially and Ethnically Diverse Medicare Beneficiaries. It was developed in collaboration with the Disparities Solutions Center at Massachusetts General Hospital and the National Opinion Research Center (NORC) at the University of Chicago as part of the CMS Equity Plan for Improving Quality in Medicare. The guide is designed to assist hospital leaders and stakeholders focused on quality, safety, and care redesign in identifying root causes and solutions for preventing avoidable readmissions among racially and ethnically diverse Medicare beneficiaries.
