The COMPASS consortium partners developed an extensive set of training materials used throughout the project, along with additional tools reflecting on lessons learned. While these resources can be used by teams implementing this model of care, they were primarily developed for training and ongoing coaching and should not be considered exhaustive (e.g., there is no information on recruiting practice sites). A significant amount of customization is needed, and they must be supplemented with additional pieces developed by and for individual practices.
For a more comprehensive overview of the model of care and implementation methods, please refer to the COMPASS Intervention Guide.
The intended audience for these materials is a generic health care organization; however, they were developed for – and thus are meant for – organizations that:
- Have some knowledge of patient-centered medical home.
- Have implemented an electronic health record.
- Have demonstrated experience and ability in quality improvement efforts.
- Have some knowledge of, or strong interest in, addressing behavioral health in primary care, and in connecting with local behavioral health services around patient care.
