Access to Care Improvement at Four Medical Groups

Acute Myocardial Infarction (AMI), Improved Efficiencies in Initial Reperfusion Treatment at Park Nicollet Health Services

Park Nicollet Health Services selected primary PCI as its preferred reperfusion strategy and has ED physicians initiate the primary intervention without waiting to consult a primary physician or cardiologist. The process change reduced median door-to-balloon time from 88 to 61 minutes.

Advanced Access Changes to Improve Mammography Waiting Time & Rates at North Clinic

North Clinic applied advanced access changes to improve mammography waiting time. The waiting time for an appointment decreased from 40 days to 18 days; the rate of patients up-to-date for a mammography screen increased from 72% to 83%. (10/01)

Advanced Access in a Multi-Specialty Group (RiverWay Clinics)

RiverWay Clinics applied advanced access methods to patient appointments in the OB/GYN department and reduced waiting times from an average of seven days to less than three days. Patient satisfaction scores increased 20% after the changes were made. (1/03)

Anticoagulation Program Development at Seven Medical Organizations

Seven ICSI member medical organizations participated in the creation of this report on the development of anticoagulation management programs or anticoagulation clinics (ACC) to improve care for patients receiving anticoagulation medications. These programs have demonstrated significant improvement in clinical outcomes when patients were managed in a consistent manner. (12/04)

Anticoagulation Therapy Improvement at HealthSystem Minnesota (HealthSystem Minnesota is now known as Park Nicollet Health Services)

HSM (Park Nicollet Health Services) increased the % of patient INRs in prescribed therapeutic range, decreased inpatient stays for patients with DVTs, & increased the % of patients in an outpatient setting through an anticoagulation initiative. (11/99)

Asthma Improvement at Family HealthServices Minnesota, PA

FHSM was given the BHCAG award in 2000 for improvements in asthma management. They increased significantly the rate of asthma severity documented, annual spirometry & peak flow monitoring & documented patient education. (3/01)

Asthma Improvement at HealthEast

HealthEast increased the rate of documentation of severity level (5% to 50%), use of anti-inflammatories (85% to 97%), clinic peak flow meter readings (46% to 73%) & patients with peak flow meters at home (23% to 27%) for patients with asthma. (12/98)

Chest Pain (Acute) Initiative at Park Nicollet Health Services

PNHS began the initiative in 1999 to shorten the length of time for patients presenting in the emergency room to get a stress test & be diagnosed. This report lists the process changes made in the ER, cardiac lab & inpatient setting. (6/01)
