HealthPartners Central Minnesota Clinics (formerly CMGH) increased both the rate of patients tested & those in glycemic control from 55% to 75% through the use of a designated nurse educator, patient registry, & other provider tools. (12/98)
HealthEast decreased the rate of patients with HbA1c >= 9 from 36% to 16% & increased the rates of HbA1c, lipid & microalbumin testing & annual foot & eye exams. Improvement efforts focused on the 6 components of Wagner's chronic conditions model. (12/99)
HealthPartners implementation of Wagner's model for improving care for patients with chronic conditions resulted in improvement in patients with HbA1c levels < 8 from 47% to 69%, HbA1c levels >= 10 from 12% to 7% & LDL < 130 from 58% to 65%. (12/99)
