ICSI, in partnership with the Minnesota Department of Health (MDH), HealthPartners Institute and Stratis Health, is providing program management and practice facilitation for five Minnesota health systems in an effort to improve their care of patients with chronic health conditions.
This program’s mission is to partner with participating clinics through June 2018 to support their efforts to organize and develop models of care that will increase their care teams’ abilities to meet patient needs. Hypertension is the primary focus of the work, but the ultimate goal is that by the end of the program, these clinics will have a general management system in place for patients with any chronic condition.
In addition to connecting clinics with a network of resources and providing other onsite support, we are also bringing participants together for four daylong workshops throughout the program.

This collaboration brings together the objectives of two complementary grants: the Centers for Disease Control State Public Health Actions to Prevent and Control Diabetes, Heart Disease, and Obesity and Associated Risk Factors and Promote School Health (CDC-RFA- DP13-1305) and the Quality Improvement Network – Quality Improvement Organization (QIN – QIO) contract awarded by the Centers for Medicare and Medicaid Services (CMS).
